General Description:
CHD’s holistic approach to behavioral healthcare seeks to help individuals and families develop balance and well-being by focusing on the following S.P.E.C.I.A.L. needs: Spiritual; Physical; Emotional/Economic; Cultural; Intellectual; Associations (Social); and Love for Self & Others. The Family Support Specialist serves as an integral part of the behavioral health team by providing resources and advocacy to help empower and engage clients to meet their S.P.E.C.I.A.L. needs. The Family Support Specialist makes a difference in the day-to-day lives of individuals by providing social work services and helping to build and support positive family and community relationships. The social worker promotes behavioral health and well-being by using assessment, diagnosis, treatment, and prevention tools.
Duties and Essential Job Functions:
Develop and maintain an updated community resources manual that is easily accessible to all CHD staff.
Provide initial client intake and connect referred clients to appropriate resources to meet identified needs.
Help clients identify options and develop steps toward reaching individual and family goals.
Assist in gathering information to assess options that support the best interests of the client.
Work closely and share information with other CHD staff, as well as with other social work and behavioral health professionals within the community.
Work Experience/Knowledge/Skills:
At least 2 years experience in behavioral health or medical health care settings or in a related social services field.
Working knowledge of behavioral health and other social service community resources.
Ability to effectively present information to children, families, staff, management, and outside agencies.
Ability to prioritize multiple tasks and meet deadlines.
Ability to apply discretion and make decisions in stressful circumstances.
Strong direct service professional skills.
Must possess excellent listening and communication skills, both written and oral.
Must have proficient computer skills.
Must be self-motivated and able to work independently.
Must possess excellent time management skills.
Must be able to work well with a variety of people and have an understanding and sensitivity toward the needs of a diverse population.
Educational/Certification/Licensure Requirements:
Bachelor’s degree in social work, human services, public health or closely related behavioral health field
Master’s degree in a related field (preferred)
Licensed Social Worker (preferred)
A combination of closely related education and experience will be considered as a substitute to these
requirements.
A valid Nebraska driver’s license, good driving record, and proof of insurance that meets agency requirements.
Criminal background and child/adult abuse clearances are required.
Drug testing is required.
